The Aging and Disability Resource Center is a statewide initiative with the intent of helping to meet the rapidly aging population by serving as a single point of entry into the long-term care system. The Center is a “one-stop shop” for access to programs, services, information and assistance.

There are 3 Primary Functions and Goals of the ADRC:

1) Awareness and Information
• Public Education
• Information on Long-Term Support Options

2) Assistance
• Long-Term Support Options Counseling
• Benefits Counseling
• Employment Options Counseling
• Referral to other programs and benefits that can help people remain in the community
• Crisis Intervention
• Helping people plan for future long-term support needs

3) Access
• Eligibility Screening
• Assistance gaining long-term support services
• Comprehensive assessment of long-term support needs and care planning
• Programmatic Eligibility Determination for long-term support services
• Medicaid Financial Eligibility Determination
• "One-Stop Shop" access to all public programs for community and institutional long-term support services
